Calculators on UOGuide pages - how to add one?

I was looking for information on how to develop/add a calculator for some pages in the wiki, but I can't figure out exactly what is needed. Is there possibly an example and any more detail what to do? I was interested in an example to keep it looking like other items on the wiki pages.
I did find the example code for a calculator reference and the page that said to contact JC the Builder about adding it, but my PM to the ICQ chat has no response yet.
